Right to Access Your Records

You have the right to inspect and obtain a copy of your medical records. To request your records, call us at 978-448-9355 or email info@itsforyourlife.com. We will respond within 30 days.

Right to Request Corrections

If you believe your health information is incorrect or incomplete, you may request an amendment. We will review your request and respond in writing.

How We Use Your Information

Your health information is used for treatment (providing and coordinating your care), payment (billing your insurance or collecting fees), and healthcare operations (quality improvement, staff training, and office administration). We do not sell your information.

Right to Request Restrictions

You may ask us to limit how we use or share your health information. We are not always required to agree, but we will honor any restriction we do agree to.

Right to Confidential Communications

You may request that we contact you in a specific way (for example, by mail only) or at a specific location. We will accommodate reasonable requests.

Right to a Breach Notification

If there is ever an unauthorized use or disclosure of your unsecured health information, we are required by law to notify you promptly.

Right to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services at hhs.gov/ocr. We will not retaliate against you for filing a complaint.
